Add "AMC-QUAM" to the MODE_BW_DEFAULTS table in app.js with the same
bandwidth settings as AM (9 kHz default, 500 Hz min, 20 kHz max, 500 Hz
step). Add an <option value="AMC-QUAM"> to the bookmark datalist in
index.html.
